Check Your Medications Before You're Discharged

  • ✅ Double Check Your Meds

    Before leaving the hospital or pharmacy, review your medications to ensure they match your doctor’s orders—right name, dosage, frequency and direction.

  • 🏥 Check Your Pharmacy Plan

    Before you leave the hospital, check that your prescription are sent to the correct pharmacy. Double-check the pharmacy’s name and address with your care team. If you prefer a specific pharmacy, let them know in advance.

  • 🥗 Should this med be taken with food?

    Some medicines work better on an empty stomach, others require food.Ask your pharmacist or provider what’s best for each medication.
